My derm is being a fag about accutane dosage

Been on accutane 20mg for like 3 months, acne still pretty bad but have seen moderate improvements especially on my chin which was literally subhuman before.
With that being said the fag still only gives me 20mg tablets. Should i try ask for 40mg or just be patient for 20mg to hit the cumulative dose.

Also i could just buy off indiamart but i lowkey dont trust poopjeets, atleast for the most part
